If you are an investor or vendor and need to inform us of a change in your current contact information, please complete the Change of Address Form, sign it, and return it to the Receiver’s office along with your photo identification in one of the following formats:

1. by email to jaypeak@akerman.com; or
2. fax the form to (954) 463-2224; or
